Before you begin

This tool helps you record conversations about life history and preferences. It does not identify ADHD. The report repeats your answers without a score, likelihood band or diagnosis. You can explore support that fits the person whether or not they have a diagnosis.

Important, please read. This is not a diagnostic tool and it cannot tell you whether someone has ADHD, dementia, or any other condition. Only a qualified clinician can make a diagnosis. This reflection is an educational and reflective aid. Its purpose is to highlight patterns that may be worth discussing with a GP or care team, and to suggest person-centred adjustments that may improve comfort and wellbeing whatever the underlying cause. If you have concerns about memory, thinking, or behaviour changes, please speak to a doctor.

Whose perspective are you recording?

Wherever possible, start with the self-report version, together with the person. Most people in the earlier stages of dementia can still answer questions about their own life story, and it is always more respectful to ask the person than to talk about them. The observational version is there for when that is no longer possible, or as a second perspective alongside it.
