Giving this to a care home

Write as much or as little as you like, then get it to the people who will look after you. There are two ways, and doing both is best.

1. Email the file. Press Save to file above. A file downloads to your computer. Attach it to an email to the manager. If the home subscribes to the Personhood Profile, they open it in their system and everything you wrote appears in your own words, with nothing retyped and nothing summarised by a stranger.
2. Print a copy. Press Print and take it to the pre-admission meeting. Ask for it to go in the care file, and ask for the summary page to go somewhere staff will actually see it. This works with any home, whatever system they use.

If the home does not subscribe, print it and send them to adhdementia.com/personhood-profile. They are not charged to receive your profile, and a manager can start a free fortnight the same afternoon. Even if they never engage at all, the printed copy in your care file still carries weight in law as a statement of your wishes.
